Supported Decision-Making and Guardianship Options

Guardianship & Conservatorship 

LSS Guardians are appointed by the court to provide Guardianship and Conservatorship services to adults who need help making decisions regarding their health care, living situation and/or finances. 
 

Health Care Agency 

You can decide how to handle unanticipated medical situations by appointing Lutheran Social Service as your Health Care Agent. A Health Care Agent makes medical decisions for you if you become unable to communicate them yourself.
 

Personal Representative of the Estate 

A Personal Representative of the Estate is the person or organization you choose to settle your estate and carry out the requests outlined in your will. Designate LSS as your Personal Representative of the Estate and rest assured the requests outlined in your will are honored.
 

Power of Attorney

A Power of Attorney is a written agreement that allows you to appoint a person or organization to manage your legal and financial affairs. By designating LSS as Power of Attorney, you can ensure a trusted agent will manage the practical, financial tasks that arise if you become unable to make them yourself.
 

Supported Decision Making 

Supported Decision Making services include Health Care Agency, Personal Representative of the Estate and Power of Attorney. Services can be combined according to your needs. With LSS’ Supported Decision Making services, you can make your own decisions – now and in the future. 

 

Pooled Trust Services

A Pooled Trust is a financial investment tool used to preserve funds that will enhance the quality of life for individuals with disabilities. Establishing a Pooled Trust can help protect eligibility for public benefits while still maintaining assets from other sources.
