Care Coordination Services


Care Coordinators empower individuals to reach and maintain their highest level of physical and mental well-being. We partner with health care insurance plans to promote access to services and resources that help individuals stay healthy in their homes and communities and prevent hospitalizations. We help the people we serve understand their health conditions and empower them to access their health plan and community benefits for which they are eligible.


Our Approach

LSS Care Coordinators are a team of licensed social workers and registered nurses who are trained in a holistic, person-centered approach to care.

Care Coordinators assist individuals through transitions such as hospitalization or rehabilitation admissions and adjusting to assisted living settings. They also provide support in obtaining preventative care, medical equipment and supplies and supports for health needs such as: arranging transportation to medical appointments, filling prescriptions and making referrals.


Who We Serve

Care Coordination is available to Minnesotans enrolled in Medical Assistance with one of the managed care organizations that partner with us. We currently work with Blue Cross Blue Shield, Health Partners, Medica and UCare.

Care Coordinators are experienced in Special Needs Basic Care (SNBC), which serves 18-64 year olds with physical, intellectual and mental health disabilities and/or brain injuries.

Care Coordinators also work with adults age 65 and older who are enrolled in Minnesota Senior Health Options (MSHO) or Minnesota Senior Care Plus (MSC+) with either Blue Cross Blue Shield or UCare.
