Case Management Services

We work with families collaboratively to assess their needs, facilitate care coordination and provide education, advocacy and referral to community resources to help them achieve success.

Our goal is to help families develop housing stability and prevent and end homelessness. We have more than 30 years of experience and support more than 200 families and 800 individuals each year.

Our Approach

We are mission-driven to stabilize families. Our services are family-centered. We strive to ensure each family's success by providing case management, advocacy for families and children, educational support, and parent and tenant education.

Many families we serve have experienced significant trauma in their lives. That might include violence, abuse and neglect, severed family relationships and extreme poverty. We work with families to effectively address these challenges and achieve housing stability and personal wellness.

Our Case Managers strive to understand each parent and child and their unique life experiences and cultural background, while supporting each individual’s strengths and resiliency.
 
Families receive ongoing tenant education to help them maintain stable housing. Our Housing Specialists work closely with the tenants and property managers toward a goal of successful tenancy.
