Helping Minnesotans achieve stable housing is our primary goal in our Prevention Program at Lutheran Social Service of Minnesota (LSS). We do this by providing direct assistance when funding is available, connecting individuals and families to other organizations that can provide direct assistance and by being a resource to connect households to other support they need.

Minneapolis Area Prevention Assistance
Lutheran Social Service and City of Minneapolis Housing Stabilization Pilot Program
Apply online by filling out this application. You can also complete the application over the phone with one of our team members by calling 612.879.5266. Applications submitted online will be e-signed after submission.
Once you have submitted the application, please begin gathering all documentation required (listed below) and send information to us at email@example.com.
- Proof of income (form 1040, W2s, last 60 days of pay stubs, benefits statements)
- Signed lease
- Landlord W9 (if applying for rental assistance)
- Proof of back-rent or utilities owed (rent ledger, utility bills)
Central Minnesota Prevention Assistance
We offer a variety of prevention assistance services in Central Minnesota, including transitional housing, long-term homeless assistance, rental assistance, eviction prevention, energy assistance and more.
