Transitional Housing

What Is Transitional Housing?
Transitional Housing is for families who are homeless and need temporary, affordable housing. While in this housing, families receive on-site case management support, which includes:

  • Weekly meeting with a case manager
  • Tenant Education
  • Housing search assistance
  • Information and referrals

Lutheran Social Service operates two Transitional Housing Programs, one in Hennepin County and one in Anoka County.

Hennepin County

  • The Family Program, located in South Minneapolis, serves one or two parent families with up to three children.
  • Families may stay in this program for up to twelve months.
  • All referrals for the Family Program must come from Rapid Exit. Families interested in applying for the program should first call Rapid Exit at 612.348.9875.
  • To be eligible for this program, families must have stayed in a Hennepin County shelter at least two times in the past 12 months.

Anoka County

  • To be eligible for the program, families must reside in Anoka County.
  • Abaku Transitional Housing is located in the city of Blaine and serves one- or two-parent families with up to four children.
  • Families in Abaku may stay in the program up to one year. Families in this program may also receive six months of support services after moving into permanent housing.
  • Referrals for Abaku Transitional Housing must come from an Anoka County Income Maintenance Housing Advocate. Families interested in applying for this program should call 763.783.4943.
