Care Coordination
Care coordination provides
assistance in assessing the needs, identifying resources to meet
care needs, and monitoring ongoing care and related services.
Care coordination is beneficial for the individual being
assisted as well as for family and friends who are engaged in
the overall care of an individual.
How can a Care Coordinator
help?
Lutheran Social Service
professional staff work with other social service organizations,
financial institutions, legal representatives, care providers
and others to assure the well-being of an individual. Following
are typical responsibilities of the Care Coordinator:
- Assist in identifying problems
and solutions
- Screen, arrange and monitor
in-home support services
- Act as liaison to distant
family members
- Advocate for proper medical
care
- Communicate with medical
professionals
- Complete medical and insurance
benefit forms
- Find professional services as
appropriate
- Evaluate long-term care
solutions
- Arrange for moving an
individual who needs other care or housing options
- Advocate on behalf of the
individual for the most advantageous living arrangements
- Work with the individual to
develop a plan of care.
What is a Care Plan?
A care plan identifies and
describes:
- The individual's needs and
goals
- Available services in the
community
- Options available to the
client and/or family.
