When LSS is appointed to be the guardian or conservator for someone unable to make decisions independently, LSS becomes the case manager.
LSS case managers make regular visits and meet with medical personnel and other caregivers so that appropriate medical and care decisions can be reached. And it may mean the case manager's orchestrating specific services that a client requires.
In addition, LSS can be
appointed to pay bills and to assure the proper management of an individual's financial and other real assets.
LSS offers Guardianship and Conservator Services in several regions of Minnesota.
