Disclosure Form for Bankruptcy Pre-Discharge Education Clients

If you are married and both are filing for bankruptcy, you must complete and submit separate Disclosure forms.

Welcome! We understand that you are here because you have filed for bankruptcy and are required to receive education before your bankruptcy may be discharged. Please read the following statements carefully so that you will understand the purpose of and procedure for the education session.

This agency has over 20 years of experience in providing financial education. Our role is not to be judgmental, but to provide assistance. This course is designed to help you develop sound financial habits, build a budget that works, learn healthy use of credit, and assist you in a return to financial well being. Upon completion of the course you will be provided the certificate that will be required to discharge your debts under the new bankruptcy law.

This agency is a member of the National Foundation for Credit Counseling ("NFCC".) The NFCC has high standards for quality credit counseling and financial education, and this agency complies with those standards. In addition, this agency is accredited by the Council on Accreditation ("COA"), an independent third-party organization that reviews and monitors entities that provide social services. We are a non-profit agency. We are organized and operate in accordance with Section 501(c)(3) of the Internal Revenue Code.

The consumer credit counselor or educator conducting the education course has been trained and certified in accordance with the NFCC standards. This course is designed to provide you with information about financial management.

To help cover the cost of providing this session to you, this agency charges a fee of $40.00 per individual. This fee may be paid in the form of debit card, cash or money order. Services are available for free based on a client's ability to pay, under the following circumstances: 1) the attorney is filing pro bono; or 2) the majority of income is from disability and the family income is no more than 200% of federal poverty level. In either case, documentation MUST be provided.

This agency does not pay or receive fees or other consideration for the referral of students to its debtor education courses.

Your counselor may provide a list of referrals for community resources, affordable housing, consumer legal advice, bankruptcy legal counsel, lending or other financial institutions. You are under no obligation to use these referrals, and you are always free to choose your own providers and services

I hereby authorize LSS Financial Counseling Service to provide to an attorney or other third party, upon my/our request, copies of counseling forms and/or certificates.
