    My Life, My Choices

    Jenna could live in a group home, but through an option called Consumer Directed Community Support (CDCS), her parents were able to design care plans based on Jenna's unique needs. The plan not only costs less than traditional care it allows Jenna to live her dream of independence.

    “My Life, My Choices” will be the next chapter in services for people with disabilities. We need your voice now to shape important priority-setting conversations for Minnesota.
