Adopted Adult Packet
Please complete and return this packet to LSS if you are an adopted adult (over the age of 19) interested in searching for your birth parent. If you are under the age of 19, you will also use this form, but your parents will need to give you permission for this search.

Birth Parent Packet
Please complete and return this packet to LSS if you are a birth parent interested in searching to initiate contact with your son/daughter or with your child's adoptive parents' (if the child is under the age of 19.)

Background Packet
Please complete and return this packet to LSS if you would like non-identifying social and/or medical information about your biological family. This information was gathered at the time of your birth.

Release of Information
Please complete, notarize and return this form to LSS if you would like to release identifying information to your son or daughter if you are a birth parent; or to your birth parent if you are an adopted adult. Once placed in the file, this form authorizes LSS to release the identifying information specified to the individual listed if and when they contact LSS in the future.

Post Adoption Request
Please complete and return this form to LSS if you would like to check whether your file has been updated. This could me from your son or daughter if you are a birth parent; or if you are an adopted adult, from your birth parent or a sibling.
